To apply for a B-Form,  NADRA  requires the following:   

Basic Requirements   

  • Valid CNIC or NICOP of at least one parent   
  • Birth certificate from the Union Council   
  • Child’s recent photograph   
  • Digital signature of parent   
  • Biometrics (age-wise, explained below)   

Biometric Rules Based on Age (2025 Update)   

  • 0–3 years: No biometrics required   
  • 3–10 years: Photograph + iris scan (if available)   
  • 10–18 years: Photograph + fingerprint + iris scan   

For Children Born Abroad   

  • Foreign birth certificate   
  • NICOP of parents

How to Make B-Form Online (Pak ID App Guide)   

Follow these steps to apply for an online B-Form:   

Step 1 – Install and Open the Pak ID App   

Download the Pak ID mobile application (Android/iOS).   
Log in or create an account.   

Step 2 – Choose the Service   

Go to:   
Issuance of Identity Document → Child Registration Certificate (CRC)   

Step 3 – Add Parent’s CNIC Number   

Enter the 13-digit CNIC of the applying parent.   

Step 4 – Review Auto-Filled Details   

Your personal information will appear automatically.   

Step 5 – Upload Required Documents   

Add:   

  • Child’s photo   
  • Parent’s digital signature   
  • Birth certificate image   

Step 6 – Add Child’s Details   

Enter:   

  • Child’s name   
  • Date of birth   
  • Place of birth   
  • Parent names   

Step 7 – Verify Biometrics   

Fingerprint verification of one parent is required.   

Step 8 – Submit Application

When uploading a child’s photo:   

  • Use a clear front-facing picture   
  • White/light background   
  • No cap or covering
